Main Purpose/Job Summary

Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, MSF protocols, values and universal hygiene standards to improve the patients/beneficiaries health conditions.

General Accountabilities:

  • Apply medical knowledge and skills to diagnose and prevention. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols.
  • Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood.
  • Follow up the evolution of the hospitalized/IDP patients, through daily visits , consultations and examinations, prescribing the necessary treatment following MSF protocols, deciding whether they can be discharged or transferred to other departments – in collaboration with other doctors- and informing their family about the patient’s evolution. 
  • Check and control the rational distribution of medicines and equipment under his/her responsibility and take care the quality, disinfection and sterilization of the medical material. Assure general compliance with standing hygiene standards.
  • Participate in the collection and analysis of epidemiological data, checking its validity and informing the line manager or project coordinator about any problem or complication of the patients illness, medical error and monitors the proper functioning of the department, equipment or material. • Ensure ongoing training of the medical/paramedical multidisciplinary team in order to optimize the quality of care. 

  • Knows and ensures all MSF medical protocols are followed and implemented, checking universal precautions are followed at all times and reducing bio-hazard risks and improving infection control. Ensures professional confidentiality is respected. 

  • Manage the team under his/her direct responsibility according to MSF HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, directly participating in emergencies and on calls if necessary.

 Specific Accountabilities – Field Focus

MSF operates a standalone 45-bed county hospital, OPD, 1 PHCC (Pieri), and 3 CBC sites (community-based health care for SV) at Pieri. The project focuses on delivering preventive and curative health care services, including Emergency medicine, SRH BEmONC with comprehensive SGBV care), paediatrics, adult IPD, ICU, TB/HIV, Outbreak response and ensures emergency referrals to MSF Bentiu Hospital (Surgeries, WW care and C-Sections). Additionally, we provide care for snakebites, palliative care, NTDs and NCDs while offering WASH and infrastructure support for all supported health structures. With an ongoing Cholera outbreak the MD will work both in the Cholera treatment center as in the general hospital.

Specific responsibilities in Lankien Project

  • Perform clinical assessment of all patients seen and prescribe the right treatment plan in accordance with MSF guidelines and protocols 
  • Work in close collaboration with clinical officers with the aim to facilitate early diagnostics and effective treatment of common infectious diseases like, Kala-Azar, tuberculosis, HIV, Leprosy, Cholera and follow infection control measures in hospital. 
  • Ensuring the implementation and standardization of MSF clinical policies, tools and hygiene. 
  • Keeping patients and their families informed of the illness and the treatment to follow, checking that they have been understood. Ensuring professional autonomy and confidentiality are respected and a good attitude and proper bedside manner towards the patient and their caretakers and the rest of the medical team. 
  • Support maternity ward in handling difficult cases and maternity patients e.g., women with co-morbidities, sick neonates 
  • Support PMTCT activities and health education activities in the ANC department.
  • Critically analyse epidemiological data for the respective departments and contribute to the Monthly Medical Report. 
  • Collaborate with other MDs in improving clinical governance, by leading case discussions and mortality reviews.
  • Critically analyse consumption against forecasted monthly consumption (FMC) and contribute to the development and adjustments of the new FMCs which are the basis for the International Medical Order. 
  • Ensure, in coordination with the Nursing Activity Manager an efficient medical equipment management, and monitor the rational use of them.
  • Experience with minor procedures such as abscess drainage, suturing, plaster making, wound care, insertion of chest tube, etc… is desirable.
  • Check all administrative procedures related to patients’ follow-up (individual card filling, registers, paper exit, discharge, transfers...) are correctly carried out according to MSF procedures. 
  • Monitor correct implementation of immunization program in children admitted to the inpatient wards.
  • Provide 24-hour on-call shifts covering all departments of the hospital and Cholera Treatment Center.
  • Cover for the other MDs and COs in their absence: Support activities in paediatric, general adult ward, ICU, Maternity, Neonates, isolation and ER.
  • Able to support Pieri PHCC remotely with clinical case management on the phone Supervise and train clinical officers’ on-job and contribute to performance evaluation for them as may be requested by Clinical Lead. 
  • Organize and hold formal and documented teaching sessions for both Clinical team and paramedical team, as well as the bedside coaching. 
  • Present difficult cases to telemedicine for more advise and Support.
  • Participate in the weekly medical meeting as requested by Clinical Lead Other duties as requested by the line manager 
  • Flexibility is required to meet the changing needs of the project.

Requirements

  • Education Medical degree and diploma in Tropical Medicine
  • Experience Essential: Experience in tropical medicine, internal medicine, emergency medicine Desirable: Experience within MSF Desirable: Cholera/outbreak response, HIV/AIDS/STDs, TB, general practice, general medicine or minor surgery, paediatrics &neonates, obstetrics, minor surgery and nutrition. Must be able to work in a large multi-cultural team, in a low resource setting, and under challenging environmental conditions. Must be prepared that insecurity may restrict personal freedom. Good communication skills and patience with local culture and relatively low-skilled staff. Flexible, team player, stress resistant and ready for challenges
  • Languages · Essential local language (Nuer), desirable mission working language (English).
  • Knowledge · Essential computer literacy (word, excel and internet)
